Week 4 - Hidden Input and Submit Buttons

Fourth Week Homework:

Design a simple form that makes use of at least two HIDDEN input fields and a graphic in place of the SUBMIT button. Test your form using either the mailto option or a mail form script. Make sure your form displays correctly and the output is what you expect it to be.

Here I am using the same form as in Week 2, but have added some hidden inputs and the submit and reset buttons. I've researched my cgi instructions for my web server and added appropriate instructions in the hidden inputs.

I liked this form, but the image submit button does NOT work, so I added a similar one to my web site as a guestbook with a regular submit button and it DOES work, go figure. You can see it HERE.
