Third Week Homework: Design a simple quiz using one or more images, a pull down list or a scrolling list with at least four options, and a TEXTAREA input box. Test your form using either the mailto option or a mail form script. Make sure your form displays correctly and that the output is what you expect it to be.

This homework is a practice exercise for your own use and will not be graded.
